How Does My Child Become a Patient?


FOR ALL NEW PATIENTS


We want to make sure we are taking the best possible care of your child. It is very important that we document the legal authority and permission to treat any new patient. For that reason ALL new patients MUST be accompanied by a parent or legal guardian at their first visit. This is true for newborns and patients transferring into our practice.


For Newborns



In the hospital your baby will be cared for by one of the pediatric hospitalists. When you are ready to be discharged, the hospital staff will call our office to schedule your baby's first appointment. We like to see new babies within 2-3 days of leaving the hospital to assess for complications from feeding problems, jaundice, common newborn infections, etc. If the hospital does not contact our office for you, please call the office to schedule your baby's appointment as soon as you arrive home.

For New Families Transferring to Us


In order to provide the best care for your child's we must be able to review their previous medical records. We can provide you with a records release form to have your child's records transferred from your previous provider or you can download it here. Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your childrens medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.

When you arrive for you child's first appointment with us, please arrive at least 15 minutes early as there will be some additional paperwork we will need to ask you to complete. It can be very helpful to complete our Patient Demographics form ahead of time and bring it with you to the first visit.
 

If you have not already done so, please sign a records release form and provide it to your children's previous medical provider.
