Sports Physicals: Participation in sports at the high school level requires documentation of an appropriate history and physical examination. You may download the VHSL form and complete the Medical History portion prior to your appointment.
School Physicals: Entrance into Head Start, public preschool or kindergarten requires documentation of immunizations, health history and physical examination on the School Entry Form. Many day care and private preschools also use this form.
ADHD: If your child is to be evaluated for attention problems or hyperactivity we encourage downloading the Vanderbilt Teacher Assessment Scale form for each of your child’s teachers and Vanderbilt Parents’ Assessment Scale.
